Hypercalcemia and HTN
A 36-year old man has mild hypercalcemia. The PTH is 105 and parathyroid sestamibi imaging is normal. Does this patient have primary hyperparathyroidism or familial hypocalciuric hypercalcemia? Will urinary calcium excretion make the diagnosis? How sensitive are these tests and how can you confidently distinguish between the two if you want to send the patient for parathyroid adenoma exploration?
